ACCESS Health International, in collaboration with Christian Medical College, Vellore, is pleased to announce an upcoming workshop on care coordination, a significant healthcare challenge.
Millions of patients, particularly within public health systems, continue to face fragmented care pathways, delayed diagnoses, inadequate follow-up, and high out-of-pocket expenses. This workshop will serve as a platform to design practical, scalable solutions that improve how care is delivered, integrated, and experienced across the continuum, from primary to tertiary levels.

Submission & Selection Process
Interested participants must submit their ideas via the structured Google Form. The form includes sections on applicant details, problem statement, proposed solution, implementation plan, impact, and scalability.
