ACCESS Health Program Manager Jean Galiana interviewed the founder of Partners in Care, June Simmons, as part of a study on elder and long term care. Simmons described the focus of the Partners in Care program and the reasons for its success.
Partners in Care collaborates with health providers and health organizations to build, test, and implement innovative models of care delivery. They focus on improving health outcomes and access to care, while doing so at a lower cost. Partners in Care collect patient centered data based on home and community interventions. Simmons describes how her company works with health providers to offer solutions to the challenges of elder care. They implement their programs around the country through training and consulting.
Collaborations with Partners in Care have resulted in greater access to preventative services. Their proposed interventions have reduced emergency department usage, hospitalizations, and nursing home admissions. These outcomes contribute to improved quality of life for the patients who participate in the programs.
